About the Position
The primary responsibility of a Field Supervisor is to ensure that our clients are receiving the appropriate services and that our Home Care Aides are following the established plan of care. The expectation is that each Field Supervisor will spend the majority of their time visiting clients. Visitation times will vary depending on the Home Care Aide’s schedule. As such, a Field Supervisor will be expected to work some evenings and weekends. Since employees have the flexibility to set their visitation schedules each week, this 40-hour-per-week position does not warrant overtime.
Responsibilities
- Responsible for approximately 180 clients
- Conduct Monthly calls and Quarterly visits to each client
- Collect caregiver task sheets every month and deliver task sheets to caregivers.
- Collect and deliver task sheets to Caregivers
- Conduct annual performance evaluations of each assigned caregiver
- Taking and completing caregiver applications
- Update work logs
- Report changes in clients’ health or living conditions
- Answer all phone calls and emails in a timely, professional manner
- Assist co-workers and other departments as needed
- Attend training sessions and department meetings
- Understand and promote all AUAF, MCO, and IDoA regulations and standards, including assisting HCAs in proper EVV clock-in/out procedures
- Review each client’s file once per quarter to ensure that information is complete and the Plan of Care is current.
- Follow all IDoA and AUAF policies and procedures
- Help and support coworkers in their absence
- Report directly to the department Manager and be under the direction of an Auditor
Conduct an on-site visit at the client’s residence once per quarter or as necessary when significant changes in physical and/or environmental changes have been reported. There are numerous reasons for this visit:
- Ensure HCAs are compliant with service schedules
- Complete home safety evaluations, assuring the clients’ homes are safe and clean
- The following procedures are to be followed:
- Use the T Sheets app for punching in and out
- Wear the provided AUAF Home Care shirt or jacket at every visitation
- Conduct unannounced visits to Client homes and document the details and observations made
- Conduct yearly evaluations of HCAs and recommend training if improvement is needed
- Other duties as assigned
Qualifications
Required
- High School Diploma or GED
- Authorized to work in the United States
- Valid driver’s license and auto insurance in Illinois, and a personal vehicle for transportation
- Pass state-required background check
- Ability to clearly document in English, per IDoA requirements
- Availability to work evenings and weekends
Preferred
- Prior experience in a home care or health care setting
- Fluent in Spanish and English
Training
- Complete IDoA required Homecare Supervisor Training (HOST) within 90 days of employment
- Complete 24 hours of in-service training on aging-related subjects within each calendar year
- Complete additional training as assigned by AUAF management

Physical Requirements
- Lift up to 10lbs
- Driving throughout Chicago, suburban Cook County, and the collar counties
- Sitting for extended periods of time
- Operate office equipment (copier, fax, scanner, phone, computer)
- Extended periods of time on the phone, typing, and reading on a computer screen

Pay: From $20.00 per hour
Expected hours: 40 per week
