LHH is currently recruiting for a Case Manager in Vancouver, WA. In this role you will be assisting adults with disabilities and older persons by assessing their needs and helping them obtain in-home and community resources. This is a direct hire opportunity.
Essential Functions:
- Perform comprehensive functional assessments and reassessments for clients.
- Develop appropriate care plans to assist clients in achieving identified outcome objectives.
- Implement short-term and long-term intensive service plans, including authorizing specific in-home and community services to meet client needs.
- Resolve client concerns regarding service delivery quality.
- Coordinate with providers of client services.
- Plan for and follow up on service termination.
- Utilize in-house nursing staff for case consultation, service plan development, and provider oversight.
- Maintain manual and computerized client records.
- Coordinate services with other community service providers, including mental health and disability service organizations.
Minimum Qualifications:
- Master’s Degree in behavioral or health science and one year of paid social service experience OR
- Bachelor’s Degree in behavioral or health sciences and two years of paid social service experience OR
- Bachelor’s Degree and four years of paid social service experience.
- Possession of a valid driver’s license, minimum state-required vehicle insurance, and reliable transportation.
- Knowledge of local in-home and community options and resources for the elderly and adults with disabilities and their caregivers.
- Ability to travel to and from clients’ homes and other community agencies, which might not be ADA accessible
