Position Overview:
The Outpatient Physical Therapist (PT) provides high-quality, patient-centered physical therapy services in an outpatient setting. The role involves evaluating and treating patients with musculoskeletal, neurological, and other physical conditions to help them restore function, reduce pain, and improve mobility. The PT will work closely with other healthcare professionals to develop and implement individualized treatment plans and assist patients in achieving their rehabilitation goals.
Key Responsibilities:

Patient Evaluation & Assessment:
- Conduct initial and ongoing evaluations of patients' physical abilities and limitations.
- Obtain patient history, perform physical exams, and assess range of motion, strength, balance, coordination, and posture.
- Utilize standardized assessment tools to determine functional limitations and goals.

Treatment Planning & Implementation:
- Develop individualized treatment plans based on evaluation findings, patient goals, and physician recommendations.
- Provide therapeutic interventions including manual therapy, exercise prescription, modalities (heat, cold, ultrasound, etc.), and neuromuscular re-education.
- Educate patients on proper body mechanics, posture, and self-management techniques.

Patient Education:
- Educate patients and their families about diagnoses, treatment options, home exercise programs, and strategies to prevent re-injury.
- Promote patient self-management, encouraging independent progress and long-term health improvement.

Progress Monitoring & Documentation:
- Track patient progress, modify treatment plans as necessary, and document all patient interactions in compliance with clinic and regulatory standards.
- Ensure that progress notes, treatment plans, and discharge summaries are accurate and up-to-date.

Collaboration & Communication:
- Communicate with physicians, occupational therapists, and other healthcare providers to ensure comprehensive care and coordination of treatment.
- Work with interdisciplinary teams to provide holistic care, ensuring continuity of treatment across healthcare settings.

Maintaining Professionalism & Compliance:
- Stay current with evidence-based practices, new treatment techniques, and industry advancements.
- Adhere to all healthcare regulations and facility policies related to patient care, safety, and confidentiality (HIPAA compliance).

Administrative & Miscellaneous Duties:
- Assist in scheduling patient appointments and coordinating follow-up visits.
- Participate in clinic meetings, training sessions, and professional development activities.
- Contribute to maintaining a clean, organized, and safe treatment environment.
