Job Overview:
This is a county position funded by the Hawkins County Government. Position is full-time with insurance and other benefits. Normal work schedule is 8:00 a.m.-4:30 p.m., Monday - Friday.
Under general supervision, a Social Counselor 2 conducts home visits, provides case management, and makes/accepts appropriate referrals for families eligible through the CHANT (Child Health and Navigation in TN) and CSS (Children’s Special Services).
Key responsibilities:
• Employee serves clients in our CHANT and CSS programs.
• Employee will maintain expected average monthly caseload of clients for home visiting and case management services provided through CHANT and CSS.
• Employee will establish plans of care that reflect involvement of the client/family in its development according to program policy guidelines.
• Employee will act as a liaison with medical services, may attend Individualized Education Plan/Individualized Family Service Plan (IEP/IFSP) meetings with school/other departmental agencies/providers when appropriate, will assist families with appeal process if denied services by insurance, will seek out medical and community resources to meet specific client/patient needs, will make appropriate referrals, and will counsel parents on the need for yearly physical exams, immunizations, and the appropriate use of health care services
• Employee will perform appropriate documentation within 48 hours of home visits both in the record and with creation of actual encounters, maintaining up-to-date databases for all patients, collecting pertinent and program required information to enroll and follow clients/families, assessing clients’ readiness for case closure and discharging patients from care as appropriate.
Minimum qualifications: (Education and Experience)
Graduation from an accredited college or university with a bachelor’s degree AND one year of professional experience in one or a combination of the following: health care for children, adolescent, or geriatric populations; social work; or social or psychological counseling, including, but not limited to, mental health, correctional, or behavioral rehabilitation counseling; or one year of experience in early intervention and/or service coordination with individuals with disabilities in an educational, social services or medical setting; or one year of teaching early childhood education or assessing individuals with disabilities.
