ChristianaCare is currently recruiting for a part time, 40 hours bi-weekly, Emergency Medicine Clinical Pharmacy Specialist to work day/evening shift rotation.
In this position, you will collaborate with healthcare providers, assess drug therapy, and provide medication-related care to achieve measurable outcomes.
Principal Duties and Responsibilities:
- Participate in acute patient care as the pharmacy resource in the emergency department, including trauma, stroke, cardiac arrest, and other unstable patients
- Assist healthcare professionals in discussions with patients and/or family members to help make informed decisions regarding pharmacotherapy options
- Provide drug therapy related education to ED team members
- Review the patient medication history to determine which maintenance medications should be continued during the acute illness
- Assist with medication reconciliation for ED patients at the time of admission, transfer, or discharge to home or facility
- Evaluate drug therapy for appropriate indication, dose, drug interactions, drug allergies, and monitor the pharmacotherapeutic regimen for effectiveness and adverse drug events (ADEs), and intervene as needed
- Provide routine stewardship activities targeted at anti-infectives and other medications, including those that may be high risk for adverse events, high cost concerns, and inappropriate utilization (e.g., factor products, anticoagulants, sedatives, acid-suppressive therapies)
- Appropriately convert/manage medications per P&T approved pharmacist policy (e.g. IV to PO and renal dose adjustment)
- Participate in the investigation and resolution of missing dose issues in the ED when needed
- Evaluate appropriate laboratory tests necessary to monitor and support the patient's drug therapy regimen when needed
- Coordinate the development and implementation of emergency medicine focused drug therapy protocols, guidelines, order sets, and/or care pathways to maximize benefits of pharmacotherapy
- Prepare and present drug therapy monographs and formulary reviews to the Pharmacy and Therapeutic committee for medications used in ED and critically ill patients as needed
- Participate in reporting ADEs to institutional committees and national programs (e.g., the FDA MedWatch program)
- Participate in investigational drug programs as requested
- Provide an interprofessional experience in training and mentoring pharmacy students, PGY-1 and PGY-2 pharmacy residents through experiential ED rotations
Education and Experience Requirements:
- PharmD from an ACPE-accredited School of Pharmacy required
- ASHP-accredited PGY1 Pharmacy Practice Residency required or equivalent experience (3 years of acute clinical pharmacy experience)
- ASHP-accredited PGY2 Pharmacy Residency (Emergency Medicine) required or equivalent experience (3 years of in a Level I trauma center or Emergency Department) in addition to PGY1 residency or equivalent)
- Board Certification (example, BCEMP) required within 2 years of hire
- Current licensure (or eligible to reciprocate) in the State of Delaware
