Job Title: Social Worker Location: Tucson, AZ 85723 Duration: 3 to 12 Months Shift Schedule: 40 hrs./week. Start Date: July 2024 Responsibilities: 1.Perform Biopsychosocial Assessments. 2.Assesses patient and plan appropriately to assist with social determinants of health 3.Suicide Risk, Mental Health, and Crisis Assessment and intervention. 4.Document safety plan and other plans of care and care given in patient's medical record. 5. Assist with discharge planning for veterans who need resources when going home, such as home health consults. 6. Reviews all information in the electronic medical records pertinent for social work services. Qualification: Education: Graduate from a nationally accredited school with an MSW. Certifications: MSW/LCSW, BLS Experience: Minimum of two years' experience as a LCSW.
