Midway homes is seeking a compassionate and dedicated Direct Support Professional (DSP) to join our team and provide high-quality home care services in compliance with the 245D Home and Community-Based Services standards. The DSP will support individuals with disabilities in their homes and community settings, ensuring their health, safety, and well-being while promoting independence and community integration.
Key Responsibilities
Midway homes is seeking a compassionate and organized Direct Support Professional (DSP) to join our team. This role includes providing high-quality home care services in compliance with the 245D Home and Community-Based Services standards, as well as performing essential office work to support our operations. The DSP will support individuals with disabilities in their homes and community settings, while also handling administrative tasks to ensure smooth service delivery.
Key ResponsibilitiesDirect Care and Support:
- Provide person-centered care and support to individuals with disabilities in accordance with their Individual Service Plan (ISP).
- Assist with daily living activities, including personal hygiene, meal preparation, medication administration, and mobility support.
- Support individuals in participating in community activities, recreational programs, and social interactions.
Health and Safety:
- Monitor and document the health and well-being of individuals, reporting any changes or concerns to the appropriate medical personnel and supervisors.
- Administer first aid and CPR as needed in emergency situations.
- Ensure a safe and clean living environment by following safety protocols and reporting any hazards.
Documentation and Reporting:
- Maintain accurate and up-to-date records of services provided, including daily logs, incident reports, and progress notes.
- Complete required documentation in compliance with 245D standards and company policies.
- Participate in regular team meetings and contribute to the development and review of Individual Service Plans (ISPs).
Training and Compliance:
- Complete all required 245D training modules, including orientation, annual training, and specialized training as needed.
- Stay informed about changes in 245D regulations and best practices in home care services.
- Adhere to all company policies, procedures, and ethical standards.
Communication and Collaboration:
- Communicate effectively with individuals, their families, and other members of the care team to ensure coordinated and comprehensive care.
- Advocate for the rights and needs of individuals, promoting their self-determination and participation in decision-making.
- Collaborate with other DSPs and supervisors to share insights and develop strategies for improving service delivery.
Office Work and Administrative Tasks:
- Assist with scheduling and coordinating care services, ensuring that all shifts are covered and any changes are communicated promptly.
- Manage and organize client files, ensuring all documentation is current and compliant with 245D standards.
- Prepare and submit billing and reimbursement paperwork accurately and on time.
- Answer phone calls and emails, providing information and support to clients, families, and staff.
- Assist with onboarding new employees, including coordinating background checks and training sessions.
- Participate in office meetings and contribute to administrative projects as needed.
Qualifications
- Education: High school diploma or equivalent required; additional coursework or certification in human services, social work, or related field preferred.
- Experience: Previous experience in home care, personal care, direct support, or office administration preferred.
- Certification: Current First Aid and CPR certification required.
- Background Check: Must pass a comprehensive background study as required by the Minnesota Department of Human Services (DHS).
- Skills:
- Strong organizational and multitasking abilities.
- Excellent interpersonal and communication skills.
- Proficiency in using office software (e.g., Microsoft Office Suite, scheduling software).
- Ability to work independently and as part of a team.
