(Y/N) -YES 3.5 FTEs Typical hiring profile Skill Set Most recruited for: (Travel RNs, local CNAs and sitters) Must have : candidates without these skills will not be considered for the role. Case Manager: 2 yrs Acute Case Management experience Social Worker: 1-2yrs Acute Social Worker experience (licensed in TX) Preferred or nice to have : candidates with these skills will be considered first. See above See Above and MSW List typical procedures performed on unit(s): Case Manager: Provides Case management service for both inpatient and observation patients assigned. Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team. Evaluates progression of care using InterQual throughout the episode of care. Social Worker: Works collaboratively with RN case managers and the multidisciplinary team, performs assessments and develops appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs. Engages in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
