Responsibilities:
Conduct thorough assessments of clients in their homes, focusing on mobility, strength, flexibility, and overall physical function.
Develop personalized treatment plans based on home assessments, considering the unique needs and living environments of clients.
Implement a range of therapeutic interventions, exercises, and modalities to improve clients' mobility, strength, and functional abilities within the comfort of their homes.
Provides supervision to Physical Therapist Assistants including case conferencing, co-signing treatment notes, and onsite client supervisory visits.
Provide client education, ensuring a clear understanding of conditions and treatment plans, and offering guidance on home exercises for ongoing progress.
Regularly reassess clients and modify treatment plans as needed to achieve optimal outcomes in a home-based setting.
Utilize effective pain management techniques to enhance clients' overall well-being.
Foster clear communication with clients, their families, and healthcare team members to provide personalized and effective home-based care.
Requirements:
Active New Jersey Physical Therapist License.
2+ years preferred but new graduates are encouraged to apply.
Valid New Jersey Driver's License.
Demonstrate unwavering professionalism with clients, team members, and families.
Exhibit strong interpersonal, verbal and written communication skills.
Showcase the ability to adeptly navigate as needed.
Must travel throughout Union County to service clients.
