The Licensed Master Social Worker is a member of a multidisciplinary treatment team operating under the primary therapy model. The successful candidate is skilled in addressing complex clinical and discharge planning needs for adults with severe mental illness and co-occurring disorders such as substance use. The successful candidate is flexible and embraces a sense of urgency in which hospitalization is time-limited with a desired outcome of community-integration in the least restrictive level of care. The LMSWII is able to maximize admission, treatment, and discharge planning interventions which may include community outreach for successful warm hand off transfers.
Specific duties/skills include but are not limited to:
Individual and group therapy - Provide person-centered and evidence-based interventions such as motivational interviewing
Provide clinical interventions addressing substance use disorders (Experience providing substance use interventions and CASAC desired but not required)
Case management and expedited discharge planning - Utilize working knowledge of community-based resources, referral coordination, and benefit management to support successful transition to the community
Family engagement, collaboration, and meetings
Complete required assessments, individual service plans, progress notes and other documentation as assigned
Accept/complete additional assignments based on unit, department and facility needs
