1. Assessment:
Conducts nutritional screenings/assessments/periodic reassessment of nutritional status.
Conduct income eligibility verification of clients.
2. Nutritional Referrals & Planning :
Prepares a nutritional care plan and a meal plan with the client , assist clients in improving access and linkages to other food sources and health care. Follow-up and evaluates client's progress toward the planned goal.
3 . Resources, Education & Training:
Plans, coordinates and implements education forums/workshops
for clients focusing on health and nutrition issues. Collects and analyzes data regarding client's health outcomes and food access.
Provides resources on community food and nutrition services.
4. Quality assurance and evaluation:
Assists and participates in evaluation and quality assurance of the nutritional program and
Projects. Maintain accurate and current client nutrition files and daily input of services delivered. Prepare monthly/bi-annual and annual service delivery reports
5 Other Duties:
Time and Labor : Employees must record their time accurately and in accordance to time worked. All time must be recorded within the Stratus time System.
