Description:
Essential Job Functions:
- Write and implement Implementation Strategy Plans, outcomes, behavior strategies and goals. Update data collections.
- Prepare and conduct annual and semiannual review staff meetings
- Schedule and conduct in-service staff meetings
- Maintain contact with OSG and family guardians
- Interaction duties with clients.
- Maintain contact with CDS, Community Day Services programs
- Monthly QIDP progress notes on all clients
- CIRAS reports on database
- Filing of client's paperwork
- New admission duties
- Maintain contact with RN for all clients
- Review ABC cards from the CILA and CDS
- Receive Personal Plans/Discover Tools completed by the ISC agency
- Send QIDP progress notes and physician orders to ISC monthly
- Special Activities with clients
- Maintain contact with BCBA
- Update DHS turnarounds with new ICAP scores and send to ISC agency
- Plan of correction from surveys
- Completion of packets requesting additional support or services to DHS
- Visit each CILA on time monthly. Address needs and requests from clients. Address needs of physical structure.
- Staff evaluations
- Facilitate individual linkage and transfer
- Training staff on DSP
- HR duties, interviewing, hiring, termination and discipline.
- Chair various committees
- Initiate and coordinate interdisciplinary process per service plan
- Assure participation of all team members
- Other duties as assigned
Requirements:
Basic Qualifications
- High School diploma or GED equivalent required.
- Bachelor's Degree
- One year of experience working with DD
- QMRP Training Program complete
- QIDP Training Complete
- Valid Driver's License
- Verified Background Check
- DSP Certified
Work Experience
Knowledge, Skills and Requirements
- Working knowledge of supervising, Excel, Microsoft Word, general ledger, accounts payable, accounts receivable and payroll required.
- Working knowledge of computer hardware, software programs, Medicare and Medicaid preferred.
- Must be able to satisfactorily complete pre-employment back screen exam for this position, drug testing, as well as an annual body mechanics screening.
- Data base knowledge a plus.
