Across NewYork-Presbyterian/Sloan Hospital for Women Care Coordinators are the guardians of continuity of care; they are the skilled clinicians leading the full spectrum of patient care, from pre-admission to discharge while ensuring quality outcomes. As a Care Coordinator at NewYork-Presbyterian, you'll serve as a highly valued and respected leader from day one, empowered to make amazing things happen for our patients and their families.
This position covers inpatient OB Units. This role assesses patients for coordination of care in-house, leads Interdisciplinary Rounds and leads coordination of discharge planning with the Interdisciplinary Team. The Care Manager/Care Coordinator role facilitates transition of patients to home providing all needed patient care equipment in the home.
The Care Manager/Care Coordinator role assumes responsibility for Utilization Management, ensuring insurance reimbursement for all admitted patients and completes the required clinical reviews to ensure admission criteria is met for all patients. For insurance denials of hospital stay, this role schedules Peer to Peer reviews with the hospital Providers and the Insurance Medical Director, forwards all updated clinicals to the insurance company for review and reversal of denial of payment and works closely with NYP's Managed Care Dept
This is a Monday to Friday position, 8:30am-4:30pm
This position requires weekend and holiday rotation.
Preferred Criteria - 2 years' of case management/care coordination experience in an inpatient Pediatric or OB setting
- Cardiac Care Experience
- 1 year of leadership experience in a healthcare setting
- Master's Degree in healthcare related field
Required Criteria - Bachelors degree in Nursing (BSN)
- Current NYS Registered Nurse (RN) licensure
- 3 to 5 years of health care experience in an acute care setting (Peds experience essential)
- Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical
